We understand that planning for a new appointment can be daunting. At Lifestyle & Wellness GP, we encourage our clients to prepare beforehand, using a simple 3-step approach.

Updated health summary

Prompt your GP to populate your health summary and get a physical copy of it. If you keep your own notes, bring this along with you too.

All results made available

Every piece of information from blood tests, investigations, imaging, procedures, and specialist letters is valuable. Please bring along physical copies or if they are digital, send them to us.

Remember your pills

Not just the night before, but to accompany you to your appointment. This includes both prescription and non-prescription medications.

FAQs

How can I book for an appointment?

Please register online as a patient to book for an appointment. If you are already registered, you will be able to book an appointment after logging in on our webpage.

As the premise is still in fit out, we do not currently take in-person booking or walk-ins.

Do I need a referral?

No, you do not need a referral to see Dr Hayati.

Can I nominate Dr Hayati to be my regular doctor?

Unfortunately Dr Hayati is unable to take the role of your regular doctor. We encourage that you keep your doctor for routine referrals, scripts and other typical GP-related services.

Will the doctor be on time?

Dr Hayati will endeavour to keep to time with his consultations. However, due to the nature of medical problems, this may not be possible. You will be advised by our friendly receptionist should there be a significant delay in your appointment.

How long will my consultation span?

Your consultation will typically last its whole duration. Dr Hayati will invite you in at the beginning of the consult before going over your material. Due to Medicare requirement, you will need to be present throughout the allocated time (including result interpretation and any paperwork requirement).

Can I book for shorter consultations?

Most, if not all health problems requiring root-cause and natural intervention solutions that Dr Hayati offers will require a longer consultation. We believe that education is equally important. Whilst your initial consultation for a specific problem needs in-depth detective work, the resulting follow-up consultation time will be spent on explanation, management options and exploring other investigations.

Our shorter, 20 minute consultations will be available in the future to suit the occasional client who only needs a focused, single issue follow up consultation.

What is your cancellation policy?

You can cancel or change your appointment up until 48 hours before the appointment time. If you cancel after this time, you will be charged a $50 fee.

Are investigations ordered by Dr Hayati bulk-billed?

As Dr Hayati is a fully qualified GP, investigations ordered by him is processed in the same way as if they had come from your regular doctor. Whilst most items are covered by Medicare, there may be some specialised tests which are not. In this instance Dr Hayati will discuss the matter with you during the consultation.

I don’t have a printer. Can I e-mail all of the necessary documents to you?

Yes. You can upload them here. Most files types are accepted. Unfortunately, due to the size of image files, we are currently unable to process these even if your upload is successful. In such a case please bring the original documents to your appointment.

Can I get my doctor to send the health summary, investigations and medication list to you directly?

Yes. Please get them to email us at admin@lifestyleandwellnessgp.com.au

Your doctor may need a transfer of information request form, which you can obtain once you are logged in as a user on our website, on the member page.

Can I get a summary sent from Lifestyle & Wellness GP to another doctor?

As Dr Hayati will not be your ‘regular doctor’, we do not provide a summary transfer service from our clinic to another. However, if you need a communication from Dr Hayati to a specific healthcare provider, please raise this matter at the latest midway through the consultation. If appropriate, Dr Hayati will produce a letter during your allocated consultation time.

I am one of Dr Hayati’s patients from a clinic he was working at previously. Do I need to register and prepare my medical documents like a new patient?

Thank you for your ongoing support in Dr Hayati’s medical service. Yes, we do encourage his preexisting patients to register anew, to avoid memory-based record error.

Will I remember the important details from my consultation?

Dr Hayati’s consultation style is focused on helping his clients understand their individual health journeys. He routinely uses diagrams and sketches as part of the consultation, which will be available for you to keep.

When can I access the red light therapy?

Every consultation comes with an opportunity to book for our red light therapy. This is accessed immediately after your consultation. We encourage that you speak to Dr Hayati if you have questions or concerns about this therapy.

Unfortunately the session is not transferrable and is not available for separate booking at the moment.
